Purpose of Your Position

The essential functions of your job are the following:

• Tp build a therapeutic relationship with residents.

• Meeting with residents on caseloads for 1:1 at least monthly

• Initiate residents’ STP care plan centering interventions on the specific needs of the individual.

• To deescalate residents in crisis and provide support following the crisis.

• To provide support in clinic with psychiatrist.

• Provide nonpharmacological interventions to psychotropic medication care plan.

• Collaborate with Interdisciplinary Team for residents' well-being.

• Provide ample objective documentation on 1:1 meeting, residents behaviors, progress, and any other information of note.

• Help clients identify goals and potential solutions to problems pertaining to mental and behavioral well-being

• Seek to improve communication and coping skills, strengthen self-esteem and promote behavior change and optimal mental health for all clienteles.

• Responsible for planning, implementing and facilitating outcome-based activities required within program.

• Provide assistance to help clients develop daily living skills and improve their mental, physical, or emotional health.

• Sufficient verbal and written communication skills required to perform tasksrequired.

• Must be knowledgeable and follow Title 22 guidelines.

Delegation of Authority

Program Counselor is delegated the administrative authority, responsibility, and accountability necessary to carry out your assigned duties.

Administrative Functions

• Conduct comprehensive assessment of client needs.

• Develop mental health care plan for clients.

• Assist clients and clinicians with weekly psych/clinic visits and document progress.

• Assist clients and develop individualized service plans with specific goals and objectives.

• Monthly and PRN documentation in patient record.
