We are currently looking for a Full Time Referral Intake Nurse. The Referral Intake Nurse is responsible for the acceptance, processing, and coordination of all referrals for patient care services.

* Assures quality patient care by receiving and analyzing each intake request for clinical and financial appropriateness.

* Receives and processes intake data from referral source

* Coordinates all patient services in coordination with the Director of Clinical Services and/or Case Managers

Qualifications of the Referral Intake Coordinator:

• Strong knowledge of processing clinical referrals
• Working knowledge of general computer programs (ie: Word, Excel, HealthWyse)
• LPN or RN with active license
