Under the supervision of the AVP of Admissions, the Center City Skilled Nursing Admissions Facilitator will be responsible for all the activities related to the admission of patients to our Center City Skilled Nursing Center.

EDUCATION:

Bachelor's degree preferred with a focus in Social Work, Nursing, or health related field from an accredited institution preferred.

WORK EXPERIENCE:

Minimum of five (5) years’ experience in healthcare.

Skilled Nursing Admissions related experience required.

SPECIAL EMPLOYMENT REQUIREMENTS:

Must be cleared by the PA State Police criminal history check

Knowledge of Federal and State long term care regulations preferred

Basic knowledge of the MDS (minimum data set) preferred

PA driver’s license and Reliable means of transportation required

Excellent verbal and written communication skills required for interaction with administration, staff, residents, families, physicians and representatives of payor sources.

Excellent organizational skills required

Working knowledge of the computer and associated programs is required

Must be able to demonstrate the knowledge and skills necessary to provide individualized care, appropriate to the age of the patient.

Must have knowledge of the admission criteria for a Skilled Nursing patient.

Must have knowledge of Insurance coverage requirements

Must maintain confidentiality of all contacts with residents/families.

• 85% Facilitates all patient admissions including but not limited to reviewing referrals and offering as appropriate in a timely manner, assigning beds, ordering equipment, confirming dialysis as needed, verifying insurance, obtaining insurance authorizations when required, assigning attending physician, notifying appropriate staff of incoming admission through facility protocol, completing preadmission screens and assessments, maintaining accurate bed boards and accurate bed hold list as well as working with the business office. • 5% Maintains knowledge on commercial insurance and Medicare requirements
• 5% Attends interdisciplinary team meetings to discuss status of bed availability, and update team on all tentative admissions dates and times. (E)
• 5% Maintains positive relationships with referral sources and families. (E)
