$20-$21 per hour

• Client: Male, 81 years old
• Diagnosis: ALS
• Location: Nassau County (Merrick, NY)
• Days/Times: Sunday-Monday 8am-8pm

Client ambulates independently and is becoming more SOB when ambulating. Standby and assist as needed to prevent injury/fall. Client has weakness to right LE and may require assistance at times. In addition, client uses wheelchair for long distances. Maintain Safety and Fall Precautions.

-Transfers: Standby and assist as needed to prevent injury/fall. Maintain Safety and Fall Precautions. Home Health Aides will provide the following tasks as indicated on the individualized plan of care:

Client lives with wife.

• Perform home management tasks including housekeeping, laundry, shopping, and errands
• Prepare and serve simple modified diets according to instruction and assists with feeding
• Assist with bathing, dressing and grooming
• Assist with medication as specified on plan of care
• Provide routine skin care
• Measure and record vital signs as directed
• Observe and report all changes to RN
• Demonstrate competence in performing necessary skills
• Complete all other duties assigned within the HHA scope of practice

HHA Job Requirements:

• Must have an active New York State Home Health Aide Certificate
• Must be a driver
• Must have at least one year of HHA experience
• Must be fully vaccinated against COVID-19
• Fluency in English
