Summary of Duties

Responsible for skillednursing clinical admission using independent judgment and discretion. Directsadmission process between physicians, other referral sources, and the facilitystaff to efficiently evaluate and admit residents. Develop and implementsmarketing strategies for referral source development.

EssentialJob Functions

1. Screen/assessappropriate referrals for admission and discuss potential for admission withthe staff.

2. Directand process admissions and serve as the liaison between the Directors ofAdmissions, hospitals, admitting/attending physicians, and other referralsources.

3. Developa system for coordinating and monitoring resident assessments and referrals.

4. Conductand document nursing assessments as necessary.

5. Completeand submit admission assessment reports as requested.

6. Coordinateadmissions with necessary staff to ensure a smooth flow of information andactivity.

7. Serve asa point-of-contact for referrals/inquires and conduct initial clinical reviewas indicated.

8. Process referralsand inquires.

9. Workwith other facility departments to ensure an accurate and timely admissionprocess.

10. Maintainan accurate record of available beds/units.

11. Communicatethe care needs and medical equipment needs of residents upon admission with theDirector of Admissions and assist as indicated.

12. Conductrounds and provide information to appropriate staff to ensure readiness foradmissions.

13. Obtainnecessary medical records for admission, i.e., diagnosis, physician orders,OBRA screening, etc.

14. Assumeadditional responsibilities and special projects as assigned or needed.

Responsibilities

1. Ensureadmission assessments and orders are obtained, physician to physiciancommunication occurs where necessary, and that the resident’s admission assessmentprocess is managed efficiently and effectively.

2. Ensure thatthe resident/family has an understanding of level of care, expectations.

3. Ensurethat information about resident progress is communicated as needed with staff,physicians, and referral sources.

4. Ensuregood public relations with the general public, residents, families, and guests,to promote a positive image.

5. Ensurethat residents and resident representatives/family members are oriented topayment source/rates as appropriate, billing procedures, visitor/guestprivileges, restrictions, and resident care procedures as appropriate.

6. Treatall residents fairly, with kindness, dignity, and respect.

7. Workwith residents, physicians, nursing staff in a polite, courteous, andprofessional manner at all times and provide support to residents and theirfamilies.

8. Maintaina clean and safe work environment.

9. Maintainconfidentiality of resident and organization information and respect residents’privacy and dignity.

10. Cooperatewith other staff members to achieve departmental and organizational goals;promote good inter/intradepartmental relations.

11. Act as arole model for good facility customer service.

12. Adhereto State and Federal regulations governing Long Term Care.

13. Participate in staff development programs and staffmeetings.

14. Create and maintain an atmosphere of warmth, personalinterest, and positive emphasis, as well as a calm environment.

15. Comply with organizational policies and procedures.

16. Report unsafe/hazardous conditions and/or defectiveequipment immediately.

17. Maintain a neat, professional appearance and adhere todress code.

18. Comply with Corporate ComplianceProgram.

19. Participatein facility and divisional meetings as requested.

20. Reportall allegations of resident abuse, neglect and/or misappropriation promptly toExecutive Director.

Education

Must have successfully completed anaccredited Registered Nurse or Licensed Practical Nurse program and becurrently licensed as a nurse by the State of Alabama.

Experience

Minimum of two (2) year’sexperience in case management and discharge planning.
