Allegiance Home Health and Rehab is looking for an experienced scheduler with Medicare Home Health experience. The scheduler is directly responsible for scheduling visits and communication with field staff, patients, physicians, etc. to maintain proper care coordination and continuity of care. 

Responsibilities:

• Maintain a current client roster with necessary information
• New Patient Intake Processing
• Conduct eligibility & benefit checks
• Responsible for the navigation of EMR and other acceptance portals
• Maintain ongoing communication with clinical supervisor, referral sources and clinical staff
• Staff incoming referrals, update, and monitor schedule changes
• Upload all necessary documents to the EMR system
• Participate in a monthly on-call rotation
• Perform other duties as assigned by the supervisor or clinical manager

Qualifications:

• Possess the ability to always provide top of the line customer service 
• Ability to multitask and prioritize tasks
• Home Health experience preferred
• Experience navigating Electronic Medical Record platforms and insurance portals is required
• Be proficient with Microsoft Word and Excel
• Knowledge of basic medical terminology is preferred.
