ALL CARE and COMCARE home health provider group has an immediate opening for a PER DIEM (field-based) California-licensed Occupational Therapist to work as a home visiting therapist for Baldwin Park, CA and it's neighboring areas. Preferably looking for an occupational therapist who is competent/experienced in assessment/charting, a fast learner, and has good communication skills.

This opportunity is also open to an experienced OT who is a self-starter in home health. We welcome highly motivated and positive-minded therapists who are willing to pursue the appropriate training to hone your home health practice. Responsibilities:

OT to undertake evaluations, assessments, and supervisory visits/ follow-ups.

Will coordinates and directs the home care patient's based on individual patient needs.

Cares for a caseload of home health patients by evaluating the patient for appropriateness of home health and developing the home care plan in conjunction with the physician.

Educates patients, families, caregivers and community providers to safely perform care.

Assesses the home environment for safety, infection control, and community resource needs.

Implement a plan of care.

Case coordination.

Attend team in-services.

Qualifications:

Minimum of one (1) year relevant OT experience.

Familiarity and work experience using home health software documentation application such as MyUnity or is willing to learn.

Registered as a professional therapist to work in California.

Valid California driver's license and current car insurance.

CPR Certified.

Knowledge regarding the impact of acute and chronic illness on the lives of patients and patient families in the community.

Functions with a high degree of independence.

Collaborates with other disciplines to provide care.

Has excellent interpersonal and group process skills.
