POSITION OVERVIEW

The Nurse Manager is responsible for coordinating the clinical and operational activities to ensure the delivery of quality, efficient, and effective patient centered care.

DUTIES

• Collaborates with medical director to plan, organize and direct administration of patient care based on established goals and objectives, standards, policies and procedures of the health center.
• Develop, maintain, implement, and monitor nursing practices within ACHC by working with clinic staff to maintain policy and producer manual, orienting medical support staff to policy and procedures, and evaluating performance of supervised staff.
• Responsible for coordination of on-going training and monitoring of competencies of registered nurse (RN) and medical support staff.
• Supervise inventory and ordering of medical supplies, medicines, and vaccines.
• Facilitates team based care initiatives including but not limited to daily huddle preparation.
• Monitors workflows to ensure efficiency and team based delivery of patient centered care.
• Collaborates with QI and Medical Director to coordinate quality improvement processes.
• Monitor and analyzes provider schedule capacity and works with team lead RN’s to maximize patient encounters.
• Coordinates population management strategies and interventions to assist ACHC in meeting established metrics for the medical and mental health departments.
• Provide supervision support for team lead RN’s, health coach, radiographer, and support staff as assigned.
• Provide direct nursing support and case management services for providers and patients when needed.
• Ensures departmental compliance with Joint Commission Standards .

QUALIFICATIONS

• Registered Nurse (RN) required
• Bachelor’s degree in Nursing from an accredited nursing program preferred
• One year of full-time work-related experience in agency and/or medical clinic service delivery (counseling, case management, outreach, prevention/education, )
• Experience leading teams required
