Description

SUMMARY

The Palliative Care Team Lead will provide support and oversight to a portion of the palliative care program, specifically direct support to the advanced practice providers in conjunction with and while reporting directly to the Sr. Director of Hospice & Palliative Services. In addition, this position will carry a palliative care caseload and staff open Advanced Practice Providers (APP) shifts as necessary to meet patient care volume depending on the prioritized needs of the program.

ESSENTIAL DUTIES AND RESPONSIBILITIES 

Administrative/Leadership Duties:

 • Assist in setting and developing team culture and targeted job satisfaction and support for APPs on the Palliative Care team for optimal team functions. • Weekly check-ins with every team member shared with Sr. Director of Hospice & Palliative Services (used for support, education, feedback, coordination, or troubleshooting).

• Provides coaching, support and feedback for performance improvement areas for both experienced and inexperienced providers

  • Direct patient care, consult etiquette, documentation, mentorship, professional development, recommended CEUs, etc. 

 • Coverage and oversight of scheduling in designated locations; On call schedule oversight as needed.

 • Coordination of surge needs and capacity including allocating Float and PRN providers for adequate coverage with financial sensitivity as needed. 

• Onboarding Oversight of new hire APPs

  • a. Assist in developing and overseeing onboarding schedule and implementation. 
  • b. Assist in ensuring logistical needs are completed. 
  • c. Monitors APP improvement and performance through high touch oversight and guidance (culture, expectations, frequent touchpoints). 
  • d. Point person for questions/concerns, troubleshooting, etc.

• Provides insight for performance evaluations of the palliative care team in conjunction with the Sr. Director and Medical Director, Palliative Care.

 • Supports the development of Palliative Care understanding within the Alive organization.

• Works collaboratively with any corporate support departments (IT, HR, Revenue Cycle).

• Solicits and provides feedback to Palliative Care leadership, identifying opportunities for improvement with frequent/routine contact with Sr. Director. 

• Occasional request for public spokesperson and resource for Palliative Care in the community including educational sessions.

 • Providing additional leadership/oversight during times Sr. Director is unavailable. 

 • Other duties as assigned depending on needs of the program.

Requirements

EDUCATION and/or EXPERIENCE

Master’s Degree in healthcare related field required; Leadership experience preferred; Nurse Practitioner board certified in Hospice & Palliative Care preferred; Entrepreneurial spirit and positive collaboration with leadership required to optimize Palliative Care program.
