Treatment Foster Care & Adoptions
Position DescriptionPosition Summary: Under the supervision of the Program Director or his/her designee, the Emergency Placement Clinical Social Worker provides case management and behaviorally focused services.
Duties And ResponsibilitiesEssential Duties:
  • Provide 24-hour on call phone coverage for Emergency Placement during assigned shift, remaining reachable by agency-issued cellular phone during business hours (and after hours if on-call) to assist with emergency and/or urgent situations.
  • Coordinate logistics of placement with resource families for youth placed in out-of-home placement into Emergency Resource Homes within contract-required timelines.
  • Perform home visits to meet with youth, complete placement packet and provide case management services and complete accompanying paperwork.
  • Provide transportation to school, appointments, or court for children placed in care.
  • Supervise family visits between foster children and birth family members and complete accompanying documentation.
  • Maintain frequent supportive contact with the resource family and foster child(ren) placed in the home as needed to support the success of the placement.
  • Maintain a partial Foster Care caseload.
  • Provide behavioral management training and interventions to children with trauma who are placed in foster care. Supports will take place in community-based settings, including schools, homes, and other community spaces, or in the Aldea office.
  • Conduct bi-weekly visits (at a minimum) with foster children and resource families in the home. Weekly visits (and more frequently if needed) should be completed during times of crisis or elevated need.
  • Complete mandatory documentation and reports in an accurate and timely manner.
  • Supervise resource families and resource homes to monitor compliance with state regulations and accreditation standards. Maintain foster child files, including the needs and services plan and other required paperwork according to state regulations and accreditation standards. Document and communicate case needs to the assigned County Social Worker and participate/facilitate treatment meetings, as needed.
  • Provide crisis intervention support and individual, group, and family counseling to foster children, biological families, and resource families, as needed while utilizing a client-centered, strengths-based approach and maintaining professional boundaries.
  • Coordinate services to support placements, including: recruiting Emergency Placement resource family homes, assessing foster child and resource family’s needs for each placement, acting as a liaison with county, providing therapeutic services, supporting educational needs, facilitating family contacts, and other such activities and participating in discharge planning decisions.
Agency Duties
  • Assist in planning of and participate in resource family and other agency activities.
  • Facilitate team approach and assist other staff in program/agency as needed.
  • Provide assistance and perform tasks related to program activities, including and not limited to compliance, resource parent training, special events, newsletters, and resource family development including participation in annual Resource Parent University planning and activities related to recruitment of potential resource families.
  • Actively participate in all agency required staff meetings, fundraising, and training opportunities.
  • Maintains all current licensing and/or professional certification requirements.
  • Abides by all provisions of the Aldea Policies and Procedures Manual and maintains confidentiality standards at all times.
  • Willing to perform other related duties needed as assigned.
