JOB SUMMARY

The Housing Information and Resource Specialist (HIRS) will work with clients who need information and resources that may be available through AID Atlanta or external community organizations to help stabilize housing, health and other socio-economic challenges. They will perform a screening assessment to see if clients meet the minimum requirement for immediate access to AID Atlanta case management, HOPWA/ HUD funded programs and referrals to other direct housing service organizations in the community. They will develop and update a database of housing resources and maintain direct relations with other HOPWA and non-HOPWA funded housing programs in the community to stay abreast of changes in programs or availability of services.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Follow up with the client within two business days of their initial contact with AID Atlanta to assess their eligibility for access to case management and program services via completion of the HOPWA screening tool and collection of eligibility documents, (proof of HIV status, Residence and Income).
  • Initiate electronic records for unestablished clients in the agency database.
  • Upload proof of eligibility in designated sections of client records.
  • Perform timely and accurate input of all required client information in Client Chart (data entry and supporting documentation) to ensure compliance with regulatory and contractual requirements
  • Document all work done with and on behalf of members into agency and/or funder client database system in accordance with documentation standard. (ECW, e2Fulton), etc.
  • Refer clients who meet the screening criteria to the Department of Client Services for case management and program servics.
  • Maintain client information and records in a confidential manner.
  • Establish and maintain list of potential housing resources and leads.
  • Cultivate relationships with housing providers to develop and maintain an active roster of income restricted communities, affordable rental properties, landlords and permanent housing resources.
  • Screen clients for services on an on-call (as needed) and on-going basis
  • Generate monthly program updates and HOPWA screening report

ADDITIONAL FUNCTIONS MAY INCLUDE

  • Assist Housing Manager and Associate Director of Housing in preparation for and during HOPWa audits
  • Actively participate as a member of Housing and/or HCC multi-disciplinary team
  • Prepare all mandated reporting as required by supervisor, contract agency and funding agency
  • Attend supervision and other required staff meetings as assigned
  • Other special projects, duties and responsibilities, as assigned

SUPERVISORY RESPONSIBILITES

  • This employee will not have any supervisor responsibilities.

QUALIFICATIONS:

Education & Work Experience

  • High School diploma or equivalent, some college preferred.
  • Experience with social service or housing programs, knowledge of AIDS program development and non-profit service agency preferred.
  • Strong experience in assessing client needs for housing or other social services
  • Considerable skill in developing and maintaining databases and spreadsheets in Microsoft Excel
  • Experience in grant compliance, reporting and administration in a non-profit organization is preferred
  • Knowledge of the housing-related difficulties affected the well-being and health status of individuals with HIV
  • Familiarity with housing entitlement systems and procedures.
  • Excellent organizational, written and verbal communication skills.
  • Ability to interact effectively with the client population and representatives of government and private agencies.
  • Ability to work effectively in a team environment.
  • Willingness and ability to travel to potential housing sites, as needed.
  • Bilingual English/Spanish would be helpful
