Job Summary
The RN Field Case Manager will be responsible for providing home health case management services to patients in the Mount Pleasant, TX area. The successful candidate will work closely with patients, families, and healthcare providers to ensure that patients receive the highest quality of care possible.
Responsibilities
- Conduct initial assessments of patients to determine their healthcare needs
- Develop and implement care plans for patients
- Coordinate with healthcare providers to ensure that patients receive appropriate care
- Monitor patients' progress and adjust care plans as needed
- Provide education to patients and families about their healthcare needs
- Document all patient interactions and care plans in accordance with company policies and procedures
- Participate in quality improvement initiatives to improve patient outcomes
- Adhere to all state and federal regulations related to home health care
Requirements
- Current RN license in the state of Texas
- Minimum of 2 years of experience as a home health case manager
- Excellent communication and interpersonal skills
- Strong organizational and time management skills
- Ability to work independently and as part of a team
- Knowledge of state and federal regulations related to home health care
- Valid driver's license and reliable transportation
Physical Demands
The RN Field Case Manager must be able to lift up to 50 pounds and be able to stand, walk, and sit for extended periods of time.
Work Environment
The RN Field Case Manager will work primarily in patients' homes and may be exposed to infectious diseases and hazardous materials. The successful candidate must be able to work flexible hours, including evenings and weekends, as needed.
