Registered Nurse - Home Health

Overview:
We are seeking a compassionate and skilled Registered Nurse to join our Home Health team. Advantage Pointe Home Care is a reputable home care agency committed to providing exceptional and personalized care to our clients in the comfort of their own homes. We are currently seeking a dedicated and compassionate Registered Nurse to join our team and conduct in-home assessments for our clients who reside in Palm Beach County.

The Registered Nurse will be responsible for conducting comprehensive in-home assessments, developing tailored care plans, and collaborating with our interdisciplinary team to ensure the highest quality of care for our clients.

Duties:

• Perform thorough in-home assessments to evaluate the physical, emotional, and psychosocial needs of clients on an as-needed and scheduled basis.
• Develop and implement individualized care plans based on assessment findings and client needs.
• Educate clients and their families about the care plan including how caregivers will assist with Activities of Daily Living (ADLs)
• Collaborate effectively with the in-office team about the client's needs.
• Maintain accurate and up-to-date client records and documentation.

Skills:

• Strong knowledge of non-skilled home health care
• Strong knowledge of common diagnoses of the geriatric population
• Strong interpersonal skills comprised of compassion, kindness and respect
• Excellent communication skills to effectively interact with clients and families
• Attention to detail to ensure accurate documentation and adherence to standards

Qualifications:

• Valid Registered Nurse (RN) license in Florida, BSN PREFERRED.
• Minimum of 3 years of nursing experience, preferably in home health or a related field.
• Experience with electronic health records (EHR) systems.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Reliable transportation and a valid driver's license.
• CPR certification.

Pay: $46.00 - $54.00 per hour
