We are a locally based home care agency seeking licensed clinical assistants to join our administrative team. You (the Candidate):

Job (the nitty gritty):

• Exhibit exemplary customer service to AMHC patients, referral sources, medical partners, and physicians
• Receive and process patient referrals from all referral sources and routes
  • Answer phone calls, emails, in-basket messages and faxes
• Create patient charts accurately and completely
• Determine and verify patient insurance eligibility
• Assign referrals to HCC team for scheduling
• Process patient hospitalizations
• Identify accurate home care orders and physician face to face documentation to verify home care eligibility
• Upload completed documentation to EMR system
• Problem solve and work to obtain valid face to face documentation from providers when needed
• Assist in (paid) rotating on-call coverage during non-office hour
• Assist in training new team members as directed
• Provide back-up for Intake Specialist Team absences as needed
• Successfully complete patient satisfaction calls
• Attend on-the-job and in-service trainings

PAY (salary):

• $62,400.00

SCHEDULE (the usual):

• Office hours - in person Monday to Friday, 8:30-5:00
• Participation in the after-hours on-call phone triage rotation

Qualifications

Experience:

• 2 years of medical office experience and/or direct patient care.
• Ability to provide excellent customer service
• Demonstrated ability to multi-task and manage time
• Ability to work and communicate professionally with patients, medical partners, employees, and supervisors
• Ability to work independently and as part of a team

Required:

• Current Minnesota Occupational Therapist Assistant certification in good standing required.
• Knowledge of medical terminology
• Excellent communication skills
• Good judgment and decision-making skills
• Strong attention to detail
• Technologically proficient and driven by accuracy
• Act as a role model of Advanced Medical Home Care values and mission
• Regular attendance and flexibility with scheduling necessary

Working Condition:

• Ability to sit, stand, type, see, hear, and communicate verbally, as well as in writing.
• Ability to complete job tasks within 40 hours per week as stated in offer.
