JOB SUMMARY

The Recovery Coach Liaison Position will be two-fold. First, as a Recovery Coach Liaison, you will play a crucial role in guiding, mentoring, and supporting individuals as they navigate their recovery paths. This position will assist clinicians/therapists with client education, discharge planning and appropriate documentation. The incumbent will complete all documentation and data entry activities as required.

Working as a liaison, this position will make the initial treatment referral based on identified needs of the individual. The incumbent will drive synergy between referral sources, patients, and the internal Recovery team. This position will establish strong working partnerships, respond to client inquiries, conduct assessments, and convert prospects into long-term clients.

DUTIES AND RESPONSIBILITIES

  • Build and maintain key relationships, driving brand awareness and acquiring new clients.
  • Establish and nurture relationships with existing referral sources, emphasizing long-term business.
  • Effectively communicate within the team to meet clients' needs and satisfaction.
  • Provide active relationship management to our clients and referral partners.
  • Monitors/records temperature, pulse, respiration, blood pressure, nutritional intake/output. Alerts professional staff to significant physical/behavioral changes/irregularities.
  • Provides oral/written description of individual's physical/behavioral change.
  • Observes client/patient/resident activities, recording/reporting unusual/hazardous behavior.
  • Conducts periodic rounds to ensure safety/well being of clients/patients/residents. Provides written/oral reports to describe unusual incidents.
  • Escorts/is responsible for individuals' traveling to social/recreational activities and medical appointments on/off campus.
  • Applies appropriate interventions to prevent or de-escalate inappropriate behaviors.
  • Serving as a liaison between the recovery community, the addiction treatment system, the medical treatment system, and the patient’s community, family, and social context to facilitate connections across systems of care.

Qualifications


REQUIREMENTS

  • High School Diploma or GED
  • Trained as a Recovery Coach
  • Can be C’NA / Recovery Coach
  • Bilingual a plus
  • Certified Peer Recovery Specialist (CPRS) preferred
  • Entrepreneurial spirit, high energy, and positive attitude.
  • BLS and CPR

EDUCATION

  • High School Diploma or GED
  • Trained as a Recovery Coach

CERTIFICATION, LICENSURE

  • Certified Peer Recovery Specialist (CPRS) preferred
  • BLS and CPR

KNOWLEDGE SKILS AND ABILITIES

  • Ability to provide exceptional patient care and timely documentation of services by consistently meeting optimal engagement and performance standards.

PHYSICAL REQUIREMENT

  • This position normally requires that physical demands of standing, walking, bending, lifting or performing other work requiring low physical exertion, talking and hearing on a regular basis to perform the job requirements.
