Our Palliative Care providers manage chronic debilitating illnesses and endof life issues. Provide cross coverage asneeded/requested by nursing and medical staff. Round on assigned medical hospitalist patients and on patients asrequested by facility medical staff.

Core responsibilities:

• Conducts family meetings.
• Performs consults and follow up visits as needed
• Create, direct, coordinate, and supervise the patient care plansdaily
• Conduct/order diagnostic tests, and analyze results and prescribeindicated treatments and therapies
• Round on existing hospitalist patients in multidisciplinary roundsif asked
• Work as part of an integrated team with care management, nursing,pharmacy, etc.
• Assist in patient education and goal setting
• Participate and adhere to all quality metrics and goals as establishedand directed by Adfinitas or the client/hospital
• Submit accurate and timely charges for all professional servicesperformed
• Attend committee, CQI and management meetings, as appropriate

REQUIREMENTS

• 1 years of experience as anAdvanced Practice Provider in an acute role, preferably long-term care orskilled nursing facility or similar inpatient experience required
• Graduateof an accredited school and program, respective to role (PA-C, or NP)
• Licensureto practice medicine in good standing, with the state governing body
• FederalDEA certification in good standing
• StateCDS in good standing
• Possess strong clinical skills and demonstrate competency inproviding comprehensive health care including evaluation, diagnosis andtreatment
• Flexibility and organizational skills to work autonomously
• Proven skills, knowledgebase, and judgment necessary for independent clinical decision making
