- Sonographer certification by the American Registry for Diagnostic Medical Sonography (ARDMS)
- American Registry of Radiologic Technologists (ARRT)
- Cardiovascular Credentialing International (CCI)
- Skills checklist (within 1 year)
- Certifications - certifications don’t need to be active at the time of submission but must be willing to obtain
- License- OR license doesn't need to be active at the time of submission but must be willing to obtain
- COVID card/status
- Oregon license
- ARDMS
- ARRT
- BLS (AHA)
