JOB SUMMARY
Recommend and assist access to medical, educational, social, vocational, housing, and more to support program goals. Initiate referrals to next level care, coordinate with physical health providers for whole-person care, and advocate in the best interest of clients.
RESPONSIBILITIES

• Conducts intake interviews and develops case plans inclusive of all needs, including but not limited to, housing, employment, court, substance abuse and mental health treatment and benefits.
• Carry a caseload and meet with clients as required at re-entry center or community location.
• Coordinates proposed treatment plan/placements as required by the program.
• Gathers information regarding available services within the community and updates the LINKED team.
• Maintains consistent contact with clients to remain informed on client’s progress and needs.
• Complies with all Turn BHS and program rules, policies, and procedures including HIPPA standards.
• Participate in scheduled program meetings.
• Other relevant duties as assigned.
QUALIFICATIONS
• AA level position: AA in social services field and 3 years’ experience providing social services particular to re-entry justice-involved populations.
• Bachelors level position: BA/BS in social/human services and 2 years’ experience providing social services.
• Masters level position: MA/MS degree in behavioral science field.
• Valid CA driver’s license and current auto insurance per contract requirements required.
• Experience providing justice involved and/or substance use disorder services preferred.
• Cultural competency understanding required.
• Knowledge of Microsoft Office and EHR software preferred.
