Job Description:
The Medical Social Worker provides assessments, coordination, and implementation/evaluation of professional social services to patients and their families in order to aid them in coping with social, emotional, and economic difficulties related to medical problems or which predispose to illness.
Job Duties –
- Implement standards of care for medical social work services.
- Assess psychosocial status of patients related to their illness.
- Plans interventions based on patient needs and findings.
- Participates in patient plan of care.
- Visit patient residences if need be ($0.67/mile for patient visits)
Qualifications -
- Graduate of a Master’s program or Bachelor’s program with Master’s oversight in social work accredited by the Council on Social Work Education.
- Minimum of one year’s experience in health care.
- Experience in a home health care preferred.
Schedule – 40 hours / week (Monday through Friday)
