Position Description:
Provides direct support for people in their home and/or community. Ensures each person's health, safety, and well-being by assisting them in acquiring, maintaining and improving self-help, socialization, and/or community living skills. Ensures activities are centered around a person's wants, interests and needs and adheres to established goals and outcomes chosen by each person and their support team.
RESPONSIBILITIES:
Demonstrates a person-centered practice approach by encouraging the achievement of each person's outcomes and goals and assists with implementing person-centered plans for each person they support.
Maintains a healthy balance between supporting a person’s health and safety and their self-determination. Direct Support Professionals provide help in a manner which respects a person’s right to privacy and upholds their dignity.
Completes all mandatory Home and Community Support training.
Assists each person in maintaining a healthy and safe environment. Continually practices universal safety precautions regarding infectious disease and shares health promotion information with the person they support.
Providing support in the person’s home, staff will complete or assist each person in performing household tasks such as routine cleaning and laundry, grocery shopping, and nutritional meal preparation. Also assists in physical transfers and/or ambulation if/when applicable to the person’s care needs.
Encourages people to develop and expand participation in their community and to build relationships with community members, businesses and organizations.
Provides transportation to community locations in a safe manner.
Documents daily service notes regarding each person’s progression towards goals, daily activities or care needs and uses the electronic billing system to capture attendance while providing support.
Follows proper communication procedures to inform supervisor of any changes, issues or concerns/questions regarding themselves and/or the person they support. This includes changes in staff’s availability, hospitalizations, and on-the-job injuries or environmental issues affecting health and welfare.
Maintains a professional and cooperative working relationship with family/other team members.
Demonstrates and understands the responsibilities of reporting incidents per state and federal guidelines, along with Achieva Home and Community policies.
Performs other related duties as assigned. The equivalent of a High School Diploma is required. Ability to complete one person transfers is required. One year of related experience is preferred. Reliable transportation is essential. A valid driver's license is required. Must meet Act 33 and 34 requirements and pass an FBI background check.
