How you can make an Impact

The Mental Health Case Manager is responsible for developing and implementing individual recovery plans. Provides interventions that decrease barriers to service. Documents observations and information regarding services delivered in progress notes. Assists in the development of social skills and daily living skills. The is done through direct in-home contact and linkage to community resources.

Case Manager Qualifications

  • Minimum requirement is Bachelors degree in psychology, social work or a related field, plus 1-2 years of relevant experience working with individuals with serious mental illness.
  • Must have and maintain a valid driver's license with an acceptable driving record.
