The Home Health Aide is sensitive and respectful with a passion for helping those in need. They are responsible for caring for multiple residents with various needs within an assisted living environment. The Home Health Aide provides personal care services and delegated health-related services under the direction of an RN. They deliver companionship and personalized care while promoting safety and encouraging resident independence and self-care.
· Performs personal care activities, including but not limited to:
a. Bathing.
b. Shampooing.
c. Skin care/nail care.
d. Oral hygiene.
e. Shaving.
f. Dressing.
g. Toileting.
h. Incontinence care.
i. Assistance with transfers and ambulation.
· Performs household services, including but not limited to:
a. Serves meals to residents in the dining room or apartments and may assist in preparing meals following preplanned menus.
b. Encourages residents to eat a proper diet and take adequate fluids while respecting the resident’s right to free choice. Reports changes in resident’s eating habits to the supervisor.
c. Maintains a clean, safe, and orderly environment for the residents; performs general housekeeping; and follows cleaning schedules for resident laundry, bedrooms, dining area, living space, bathrooms, kitchen, and other areas.
· Follows proper procedures in emergency situations and responds promptly and positively to resident requests for assistance.
· Aware of the location of each resident, reminds them to sign out when they leave the building, and notifies the supervisor immediately if unable to locate a resident.
· Assists residents with medication as defined in medication procedure; assists with reminders to residents who self-administer medication; follows physician orders and state laws to administer medications to residents who cannot self-administer their medications; and exhibits understanding of and follows medication policies and procedures.
· Maintains residents’ records daily in a timely manner and in accordance with company policy and procedures, documents medication distribution as applicable, observation of incidents and reports any changes in resident’s physical condition and/or behavior to nurse; observes and reports the health and emotional condition of each resident; and promptly reports all changes to the supervisor.
· Records observation of incidents and notify supervisor in any change in resident’s physical condition or emotional behavior as part of the resident care team.
· Documents cares provided in Electronic Health Record.
· Promotes personal safety and a safe environment for residents by observing infection control practices, following agency guidelines, and reporting unsafe situations to the supervisor.
· Demonstrates safe practice in the use of equipment. Does not use equipment until orientation has been provided. Notifies supervisor of educational needs.
· Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings and team conferences, as requested.
· Maintains confidentiality in all aspects of the job.
· Maintains confidentiality of resident’s situation, records, and services according to HIPAA regulations.
· Participates in required educational training programs to meet compliance requirements. Identifies learning needs to agency supervisor.
· Adheres to Accra’s professional standards and assisted living policies and procedures. Follows federal, state, and local requirements.
· Participates in on-call coverage.
· Performs other related duties and responsibilities as deemed necessary.
Requirements
· Successful completion of a formal certification training program and/or a written skills assessment test and competency evaluation.
· Be at least sixteen (16) years of age.
· Desired to have six (6) months’ work experience in a supervised setting, preferably health care facility.
· Demonstrated ability to read, write and follow a written Plan of Care.
· Good interpersonal skills.
· Dedicated, reliable, and sensitive to the needs of others.
· Excellent verbal communication and listening skills.
· Compassionate and Caring.
