Position: Per Diem RN Location: Dekalb County, Georgia (Field Visits) Job Type: Per-Diem, Flexible Hours

Responsibilities:

• Conduct thorough home safety assessments to identify potential risks and hazards.
• Obtain vital signs and perform mini head-to-toe assessments with precision and care.
• Provide compassionate support and education to patients and their families regarding health management and safety measures.
• Collaborate closely with interdisciplinary teams to ensure comprehensive care plans tailored to each patient's needs.
• Maintain accurate documentation of assessments.

Requirements:

• Active Registered Nurse (RN) license in the state of Georgia.
• Minimum of 2 years of nursing experience, preferably in home health or a related field.
• Strong clinical assessment skills and attention to detail.
• Excellent communication and interpersonal skills.
• Ability to work independently and adapt to changing environments.
• Reliable transportation and willingness to travel within designated geographic areas.
