Our Case Manager Assistant will assist the Case Manager with admission of new clients by providing program explanation, scheduling and program hours. Ensure all intake paperwork is complete and accurate and entered into the admission software. Maintain ongoing communication with clients and families about program needs and resources. Process and distribute medical records requests from external sources in a timely manner according to NCEP policy and procedure; assist with treatment planning agenda and distribute information in accordance with treatment plan or discharge recommendations. Must haves: Associates or higher degree in social work, human services, psychology and/or 2 years of experience in a healthcare or related field. Demonstrated ability to successfully collaborate and work productively as a member of a multidisciplinary team. Excellent written and oral communication skills and interpersonal skills, including the ability to work well with a diverse range of individuals. A demonstrated customer-service approach to work. Experience working with individuals with disabilities and basic knowledge of medical terminology and the ICD-10 a plus.
