Admissions Navigators (Navigators) shall process all requests for clinic, housing, recovery, and support services by communicating directly with potential clients, collecting relevant information, and presenting information to department leads to determine eligibility. Navigators shall also assist persons with applications for other public/community assistance programs as needed.
Minimum Qualifications:
One of the following education/experience levels is required:
· Bachelor’s degree in Counseling, Education, Psychology, Nursing, Social Work or related field with four years professional experience. A master’s degree in a related field can substitute for two years of professional experience.
· Associate’s degree in Health Care with six years professional experience.
· Nine years of professional experience working with in an HIV-related service.
Ability to work with people of diverse lifestyles required.
Prior experience working in an HIV service, HIV prevention, healthcare, or substance abuse rehabilitation program preferred.
Responsibilities:
· Interact with potential clients requesting services from AcadianaCares.
· Understand current availability of in-house services.
· Understand eligibility criteria of in-house services.
· Collect admissions demographic information, including insurance and income data.
· Conduct initial risk/need assessment interviews.
· Asses urgent and emergency requests/needs and promptly consult department leads to resolve requests/needs in a timely manner.
· Obtain admissions-related information from outside sources, including clinics, hospitals, rehabilitation facilities, correctional facilities, mental health facilities, shelters, government programs, and other community-based organizations.
· Assist with applications for insurance, public assistance, and community programs, such as Medicaid, Temporary Assistance for Needy Families (TANF), Louisiana Supplemental Nutrition Assistance Program (SNAP), Women, Infants, and Children (WIC) nutrition, and Louisiana Health Access Program (LA HAP).
· Consult with Health & Wellness Director to address urgent or emergency issues, seek guidance regarding determinations, and resolve department issues.
· Provide and facilitate referrals for services not offered at AcadianaCares.
· Enter all relevant data into AcadianaCares enterprise software programs for data tracking, invoicing, and billing, including scanning forms for data management.
· Present admissions information to department leads to facilitate eligibility determinations
· Communicate results directly to potential clients and transition them into service or admissions discharge.
· Maintain periodic contact with clients who are not actively receiving services.
· Reconnect with clients who dropped out of service to reassess eligibility and need.
