Abode Services, one of the largest and effective nonprofits working to end homelessness in the Bay Area, has an opening for a Wellness Specialistat our San Mateo County site.

POSITION SUMMARY: Healthcare for Homeless/Farmworker Program is a program that ensures that clients who are transitioning into a stable housing situation, especially those without supportive services, can access or continue to access medical, dental and behavioral health services after exiting homelessness in San Mateo County. This position will provide a range of care coordination services to clients transitioning out of homelessness into stable housing situations so that they can access medical, dental, and behavioral health services up to 12 months after moving into housing.

RESPONSIBILITIES / DUTIES:

  • Helping clients, in consultation with their medical providers, to develop and adhere to a care management plan.
  • Supporting clients in completing referrals (e.g., to OBGYN, BHRS, dental and other specialty services) received by their provider.
  • Helping clients establish a medical home (i.e., primary care provider) for those who do not already have one.
  • Assisting clients with scheduling and attending healthcare appointments, including delivering appointment reminders and accompanying clients, either in-person or via telehealth, to healthcare appointments.
  • Arranging transportation to healthcare appointments.
  • Providing non-medical translation.
  • Assisting clients with completion and renewal and eligibility benefits. Contractors shall work closely with the Health Coverage Unit to help clients enroll and remain enrolled in health coverage; Providing information on health and community resources to clients.

Qualifications:

  • BSW/BA Psychology with 3 years experience working with working with people with serious mental illness, individuals with co-occurring disorders and/or the homeless population.
  • Experience with street outreach a plus.
  • Must have a working knowledge of Psychiatric Disorders as well as knowledge and ability to implement the following evidence-based models: Harm Reduction, Psychosocial Rehabilitation, Strength-based Case Management and Motivational Interviewing.
  • Ability to build supportive and respectful working relationships with individuals diagnosed with a mental illness that instills hope and promotes self-determination. Sensitivity to and understanding of the special needs of the homeless.
  • Proven ability to work independently and as an effective and collaborative member of a team.
  • Excellent verbal skills. Strong organizational and time management skills.
  • Strong writing skills are a MUST.
  • Possess an understanding of and practice cultural sensitivity through open dialogue and self-exploration with diverse groups, while providing direct services.
  • Ability to effectively intervene in crisis situations, with de-escalation techniques.
  • Reliable transportation and proof of a valid and current California Driver’s License and current insurance along with a clean DMV record required.
  • Ability to work flexible hours, including some weekends and evenings.
