REPORTS TO: DD (Developmental Disabilities) Health Home Director
Position Overview:
As an integral part of the Abilities First Developmental Disabilities Health Home (DD HH) team, the Nurse Care Manager (NCM) plays a pivotal role in managing the comprehensive healthcare needs of individuals with developmental disabilities (DD). Employing a holistic approach, the NCM addresses both physical and behavioral aspects of health, overseeing the delivery of core services including care management, coordination, health promotion, transitional care, family support, and community referrals. Collaborating closely with the DD Health Home Director, healthcare consultants, and interdisciplinary team members, the NCM ensures tailored care plans that prioritize individual well-being.
Essential Functions and Key Responsibilities:
· Facilitate Collaborative Care: Lead interdisciplinary team efforts to streamline care management processes, ensuring cohesive and efficient service delivery.
· Individual Engagement: Initiate meetings with assigned individuals to explain the benefits and objectives of the DD Health Home program, obtaining informed consent for participation.
· Chart Review: Conduct comprehensive chart reviews within 30 days of enrollment to identify and prioritize individual health issues, establishing a baseline for ongoing care.
· Health Screening: Administer Metabolic Syndrome (MBS) screenings within specified timelines, coordinating with primary care providers for necessary lab assessments.
· Comprehensive Assessments: Conduct standardized health assessments, documenting findings on Health Screen forms to inform personalized care plans.
· Care Coordination: Develop individualized, person-centered care plans that integrate clinical and non-clinical healthcare needs, fostering active participation in goal setting and plan revisions.
· Documentation and Monitoring: Maintain accurate documentation of services provided, including care management monitoring and medication reconciliation interventions, adhering to regulatory standards.
· Health Education: Organize educational groups to empower individuals in managing chronic illnesses and adopting healthy lifestyles, promoting self-management and wellness.
· Population Health Oversight: Monitor health trends across the served population, identifying and addressing emerging issues through targeted interventions and education initiatives.
· Managed Care Coordination: Liaise with Managed Care Organizations (MCOs) to coordinate care for enrolled individuals, ensuring seamless transitions and continuity of services.
· Physician Collaboration: Maintain open communication with primary care physicians, facilitating referrals and assisting individuals in establishing and maintaining healthcare relationships.
· Staffing Ratio Compliance: Adhere to staffing ratio guidelines, ensuring effective management of caseloads while maintaining quality standards.
Education, Training, and Experience Requirements:
Registered Nurse (RN) or Licensed Practical Nurse (LPN) with current Missouri or compact license recognized by the Missouri State Board of Nursing. Experience in the IDD field preferred but not required.
