Job Description: Are you highly organized, detail-oriented, and passionate about making a difference in the lives of individuals with developmental disabilities? Abilities First is seeking a dynamic DD Health Home Nurse Care Facilitator to join our team. In this role, you will play a pivotal role in managing the administrative functions of our DD Health Home program.

Key Responsibilities:

• Create and maintain organized systems for managing and tracking data related to DD Health Home participants.
• Utilize various software and databases to enter, track, and manage data, ensuring alignment with contractual requirements.
• Facilitate information exchange between internal teams, collaborators, contractors, and state agencies.
• Track data on enrollments, discharges, ER visits, screenings, and other key metrics.
• Provide IT support related to DD Health Home services.
• Assist in initial meetings with eligible individuals and support general DD Health Home functions.
• Uphold strict confidentiality standards and adhere to ethical conduct in all interactions.

Requirements:

• High School diploma or equivalent.
• 3 years of administrative support experience preferred.
• Experience in the IDD field is a plus.
• Proficiency in Microsoft Office Suite and ability to quickly learn new software.
• Excellent communication skills, both verbal and written.
• Ability to work independently with minimal supervision.
• Strong organizational skills and attention to detail.
• Valid Missouri driver’s license, reliable transportation, and auto liability insurance.
• Reliable high-speed internet for remote work.
