SERVICE COORDINATOR:
Essential Functions: Service Coordinators are responsible for assisting individuals as determined within the Person-Centered Individualized Support Plans (PCISP), provide quality care to individuals, and to increase quality of life in the direction of the individual's choice. Team Lead/PC is responsible for overseeing the daily operations of a specific client residential site, or group of community service clients, and is responsible for assisting with the implementation of procedures to regulate an individual’s home and/or services. Consistent attendance and the ability to independently manage a work schedule are general qualifications for this job. The Service Coordinator oversee the daily operations of a specific caseload of individuals receiving services as part of the Family Supports Waiver and Community Integration Habilitation Waiver Programs to include the following services:
- are services that are specified in the PCISP and support learning and assistance in the areas of: self-care, sensory/motor development, socialization, daily living skills, communication, community living, and social skills. Day habilitation activities are intended to build relationships and natural supports. Services are provided in a variety of settings in the community or in a facility owned or operated by an FSSA/DDRS-approved provider. Settings are non-residential and separate from a participant's private residence or other residential living arrangements.
- are ongoing employment support services that enable a participant to maintain integrated competitive employment in a community setting. - Services are provided in order to allow participants (consumers) with developmental disabilities to remain and live successfully in their own homes, function and participate in their communities and avoid institutionalization. PAC services support and enable the participant in activities of daily living, self-care, and mobility with the hands-on assistance, prompting, reminders, support and monitoring needed to ensure the health and safety of the participant. A relative of the participant may be a provider of PAC.

Respite Care can be provided in the participant ts home or place of residence, in the respite caregiver(s home, in a camp setting, in a DDRS approved day habilitation facility, or in a nonprivate residential setting (such as a respite home).
— daily (RHS Daily) services mean individually tailored supports that are specified in the PCISP that assist with the acquisition, retention, or improvement in skills related to living in the community. These supports include adaptive skill development, assistance with activities of daily living, community inclusion, transportation, adult educational supports, and social and leisure skill development that support the participant to live successfully in his or her own home.
A Service Coordinator is required to service a minimum of at least 32 billable hours per work week. Consistent attendance and the ability to independently manage a work schedule are general qualifications for this job.

a. Ensure that the safety and health of Individuals is maintained through continuous monitoring.
b. Maintain highest standard of grooming and hygiene, bathing, toileting, oral care, shaving, dressing, and grooming
c. Provide transportation in accordance with consumer social, medical, and general developmental needs.
— Provide Dietary Options — Prepare meals in accordance with dietary plans and encourage individual successful completion of dining objectives.
— Provide Quality Care of the Individual's home - Ensure the surroundings of the Individual remain clean and in good repair through daily cleaning, laundry, testing smoke and C02 alarms and maintaining appropriate water temperatures.
— Provide Quality Medical Care of Individual
Understand and deliver delivery care for individuals in compliance with the PCISP and company procedures
Administer medications: oral as a requirement.
Monitor health related issues such as dysphasia, seizures, diabetes, etc.
Assist with medical appointments
a. Understand and handle Self Injurious Behaviors (SIB), physical aggression, spitting, biting, pinching, property destruction, PICA, verbal aggression
b. Charting and consumer documenting
Maintain consumer's records and other documentation per established procedures.
Timeliness, completeness and accuracy of oral and written communications. — Provide professional internal and external relationships, cooperation, and partnership with all ABC LLC employees, managers/leads and Individuals.
