ABA Home Health provides home care to beneficiaries in the District of Columbia.

ABA is currently seeking Full Time, Part Time, and PRN Registered Nurses to join our growing organization to provide world-class care for our clients. Responsibilities:

The Registered Nurse in collaboration with a multidisciplinary team centered on patient care need will instruct and educate the beneficiary and family about proper disease management.

Will work closely with the Director of Nursing to develop a care plan and follow such a plan as established by the beneficiary’s physician

• Assesses the physical, psychosocial, spiritual, and environmental status of assigned clients in a systematic and continuous manner to provide required client services.
• Consults with physicians, other staff, client/family, and other caregivers to formulate a plan of care and coordinate delivery of services to ensure high quality client care.
• Provides home health nursing services in accordance with the plan of care established by the client’s physician.
• Performs ongoing assessment of client's status and response to services to evaluate the plan of care and modify as necessary.
• Performs related responsibilities as required or directed.
• Initiates and revises client care plan for the Personal Care Aide (PCA)/Home Health Aide (HHA) based on client's needs and supervises the care provided by the direct care staff.
• Informs the physician and other healthcare team members of changes in the client's conditions and needs.
• Other duties can be assigned.

Qualifications:

• Registered Nurse with a valid DC MD VA or compact state license.
• One year (1) year experience as a Registered Nurse, Home Care experience preferred, experience in Oasis assessments a plus
• Ability to provide care in a variety of settings
