Requirements:

• Must be bilingual fluent in English and Creole.
• Previous experience as Senior Coordinator preferably in a home care setting.
• Bachelor's degree.
• Home Care Experience Preferred
• Demonstrated ability to provide compassionate care and establish rapport with clients.
• Excellent communication skills, both verbal and written.

Responsibilities:

• Identify and assign the proper HHA to meet client needs
• Communicate with RN-Home Care Supervisor regarding all new client assessments and reassessments
• Provide general information to the public regarding home care services
• Take note of any complaints
• Make sure all employees make their Punch In and Out in our App
• Schedule and re-schedule HHAs to new cases and other cases if necessary
• Make the first call with every new HHA hired to confirm the availability provided and any request
• Send communication forms to the insurance regarding any change or issue we may have in any case
• Make follow up calls to insurance regarding new authorizations pending
• Request TimeSheets to HHA/CDPAP that couldn't do their Punch in and out
• Communicate with the client's proxy regarding any new change in their schedule
• Provide to new HHAs hired how to do their punches in the App
• Provide to new RNs hired how to use RN PAD
