We are seeking a compassionate and experienced Social Worker to join our team working to build resilience in our communities. The Social Worker will play a key role in providing case coordination, referrals and linkages to other service providers. This role also provides other early intervention activities and educational interventions to at-risk youth, particularly African American youth. The ideal candidate will have a strong background in clinical social work, with a focus on prevention and early intervention strategies and experience providing therapy services as a registered associate with BBS.
A Greater Hope is a nationally accredited community-based organization providing health care to the community's most vulnerable. A Greater Hope provides a continuum of care that includes Child Abuse Prevention and Treatment, Foster Care, Adoption, Live Scan, Kinship Support, Domestic Violence Counseling, Supervised Visitation, Probation and Behavioral Health Care services. Our mission is to enhance the quality of life for children and families through culturally responsive integrated services. Our goal is that every client, from a 38-year-old single dad to 3-day-old twin baby girls, finds A Greater Hope.
Key Responsibilities:
Case Management: Provide comprehensive case management services, including assessment, goal setting, and coordination of services to meet the needs of program participants. Develop and monitor prevention and early intervention plans tailored to each individual's strengths and challenges.
Educational Sessions: Facilitate individual and group educational sessions on topics related to mental health, coping strategies, stress management, and life skills development. Adapt curriculum and activities to meet the unique needs and interests of participants.
Documentation: Maintain thorough and accurate documentation of all client interactions, assessments, treatment plans, progress notes, and referrals in accordance with agency policies and procedures. Ensure compliance with confidentiality regulations and ethical standards.
Resource Referrals: Provide participants with linkages and referrals to community resources and support services, including mental health treatment providers, educational programs, housing assistance, and other relevant resources as needed.
Collaboration: Collaborate with program staff, including mental health specialists, office assistants, and community partners, to ensure coordinated and holistic care for program participants. Participate in case conferences, team meetings, and training sessions as required.
The starting salary range for this position is $67,060-$69,072.
