Your responsibilities would be:
- Conduct thorough initial assessments and develop individualized care plans for home health patients.
- Provide compassionate and competent patient care in accordance with established care plans.
- Monitor patient conditions closely and promptly report any changes to the healthcare team.
- Administer medications and treatments as prescribed by healthcare providers.
- Work closely with caregivers to ensure effective implementation of care plans and continuity of care.
- Educate patients and their families on health management and preventive care measures.
- Ensure compliance with all relevant health and safety regulations.
Requirements:
- Current Florida RN License: Must hold a valid RN license in the state of Florida
- Minimum 1 Year of Nursing Experience: Preferably in home health or a similar healthcare setting.
- Demonstrated Assessment Skills: Proven ability to conduct thorough initial assessments, develop comprehensive care plans, and collaborate effectively with caregivers.
- Strong Communication Skills: Excellent verbal and written communication skills are essential.
- Autonomous Work Ability: Ability to work independently and efficiently manage time.
- Valid Driver's License and Reliable Transportation: Must possess a valid driver's license and reliable transportation for home visits.
