Department: 02191 AMG Arlington Heights Golf Rd - Immediate Care: Retail Status: Full time Benefits Eligible: Yes Hours Per Week: 37 Schedule Details/Additional Information: Weekdays 9a-5:30 Saturday 9-3 Sunday 9-2 MAJOR RESPONSIBILITIES Evaluates, diagnoses, treats, and manages acute patient illness and diseases via virtual visits, with the support of adjunctive assessment technology, and in-person visits, as needed. Obtains medical histories, conducts visual examinations (as needed and as able), assesses and diagnoses both acute and chronic conditions. Prescribes medication, therapy, and treatments accordingly. Orders, interprets, and evaluates patient diagnostics to develop patient care plan. Counsels, instructs and educates patients on physical and mental health including diet, preventive health measures, health behaviors, self-care skills, disease, treatment options, and normal growth and development. Provides care coordination including management of outbound phone calls and scheduling patient appointments. Appropriately communicates with colleagues during shift handoffs (e.g., relay status of potential patient follow-up calls, outstanding lab orders, unaddressed Epic In Basket messages, etc.). Covers lab results from orders created by APCs. Documents patient encounters in the electronic medical record (EMR) maintaining records according to established standards and guidelines. Applies clinical knowledge as requested by, and for support of, clinical engagement center team members (e.g., assistance with calls from patients, pharmacies, facilities, etc.). Provides clinical direction to engagement center nursing staff regarding the continued care of patients. Follows through with nursing staff to ensure patient receives recommended care. Integrates with and must be available to engagement center team members via chat, phone, or contact center workstation. Actively participate in quality reviews. Ability to work remotely from home in a private, secure setting with access to a nearby Advocate/Aurora Health facility/office in case of home technology issues. Maintain professional appearance and professional setting (home or work office) for all virtual Visits. Must have high-speed internet set up. Work rotating schedule around major holidays and rotating weekend schedule. Flexibility to cover shifts based on staffing needs. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards. MINIMUM EDUCATION AND EXPERIENCE REQUIRED License/Registration/Certification Required: RN, APRN and Furnishing licenses (if applicable) issued by the state(s) in which the team member practices. Position requires licensure in IL and WI. Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 21 days of hire, Nurse Practitioner certification issued by the American Academy of Nurse Practitioners (AANP) OR the American Nurses Credentialing Center (ANCC) Drug Enforcement Administration (DEA) registration issued by the Drug Enforcement Administration (DEA) Registration, and Controlled Substance Licensure (CSL) Education Required: Master’s Degree in Nursing Experience Required: Typically requires 2 years of experience. KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED Demonstrated high level of clinical proficiency, and excellent decision-making skills. Demonstrated ability to work independently and as an effective member of a health care team. Demonstrated ability to adapt to evolving technology and proficiency with the computer and the electronic medical record. Excellent communication skills. Ability to effectively collaborate with and establish/build relationships with others. Proven organizational skills and ability to prioritize effectively. PHYSICAL REQUIREMENTS AND WORKING CONDITIONS Must be able to stand, walk, bend, stoop, and twist continuously throughout the workday. Must have functional speech, vision, touch, and hearing. Hand dexterity to type, take vital signs, render emergency patient care via BLS or other measures Operates all equipment necessary to perform the job.
