Pay Range: $37.86 - $46.58 Hourly Department: Health Department Job Type: Regular Represented Exemption Status: United States of America (Non-Exempt) Closing Date (Open Until Filled if No Date Specified): The Opportunity: THIS WORK MATTERS! **When applying please be sure to follow ALL instructions under “Screening & Application”** The Aid & Assist Mental Health Consultant position is responsible for complex planning, coordination and advocacy with specialized mental health care needs who are legally/criminally exposed, with an emphasis on Aid and Assist clients. The duties of this position include consultation, assessments, and care coordination for individuals who are mentally ill and unable to aid and assist in defense of a legal case and require competency restoration services. Persons in this position will regularly communicate, coordinate, and collaborate with Courts, attorneys, jail, Oregon State Hospital (OSH), Oregon Health Authority (OHA), community based service providers, and other stakeholders regarding the needs of individuals who are unable to aid and assist. Individuals in this role will be involved in care coordination for clients and the completion of referrals. Duties include timely documentation of all client interactions, timely completion and filing of tasks required by the Oregon Revised Statutes (ORS), Oregon Administrative Rules (OAR), and Court order, and keeping program tracking up to date. As the Aid & Assist Mental Health Consultant, other duties will include: Assessment, Consultation, Care Coordination, and Referral Actions Intensive care and case coordination, direct consultation, oversee, evaluate and coordinate behavioral health services performed by internal and community providers, and conducts consultations for Coordinated Diversion and Choice eligible clients with complex needs including clients with specialized mental health care needs to ensure that client needs are appropriately and adequately addressed by service providers. Complete mandated functions and services including consultation, evaluation, needs assessment, care coordination, making referrals, and care recommendations for adults who exhibit severe emotional disturbances, substance abuse, criminal behavior and are unable to aid and assist in defense of legal charges against them. Ensure timely coordination with all partner agencies and respond to requests for intensive care coordination services. Provide services as required by legal and contractual mandate to help restore individuals’ competency to stand trial. Services required can occur while a person is in jail custody, in the State Hospital, or in the community. Interview clients for the completion of Community Consultations as ordered by the Court. Initiate, attend, and participate in regularly scheduled and urgent/impromptu staffings to collaboratively problem solve any barriers to services/discharge planning. Locate and assist with arranging specialty mental health services, as appropriate, upon request by client, physician or provider, to ensure that client needs are appropriately and adequately addressed. Exchange risk assessments for clients with physicians or providers to increase the knowledge of overall risk of specific clients who have been identified with a mental health diagnosis to determine if specialty service is appropriate under the client's treatment plan. Documentation and Meetings Regularly communicate with the Aid & Assist Team and the court regarding consultation findings, status updates related to referrals, care planning, and client adherence to community restoration orders. Communicate at least every thirty (30) days with each client receiving competency restoration at Oregon State Hospital and communicate monthly with OSH staff who are assigned to the care team of each Multnomah County client. Document all coordination efforts in the electronic health record (Evolv NX), including, but not limited to, communication with service/placement providers, member/provider contacts and meeting attendance/staffing. Complete court ordered tasks and make court filings within the statutory and/or court ordered judicial time frames. Make and document required contact with clients committed to OSH for competency restoration. Charting and Records Track client information, status, referrals, treatment needs, and internal and external partner/provider decisions and recommendations in order to report out to managed or coordinated care organizations for funding or charge-back actions, grant requirements, etc. Provide clinical recommendations to community partners regarding needs and recommendations for services needed. Collect and review medical, mental health, and social information from treatment records and medical charts, make clinical observations and present findings and recommendations at internal and external stakeholder meetings. As a newly hired Aid & Assist Mental Health Consultant , you will receive a one-time New Hire Recruitment and Retention Incentive payment of $1,500 on your first paycheck. The incentive will be prorated based on the position's FTE status. TO QUALIFY: We will consider any combination of relevant work experience, volunteering, education, and transferable skills as qualifying unless an item or section is labeled required. MINIMUM QUALIFICATIONS (REQUIRED): Education/Certification: Must meet the standards of Qualified Mental Health Professional - (QMHP - C) with the Mental Health & Addiction Certification Board of Oregon (MACBOOK) - more information can be found at MHACBO website. Current QMHP - C, and/or licensed Board of Oregon (LPC, LCSW, PHD,etc). Graduate degree in psychology or Graduate degree in social work or counseling or Graduate degree in a behavioral science field Other: This position requires a background investigation, which may include being fingerprinted. Valid Driver's License. Requires use of personal vehicle or vehicle from fleet to travel to assigned sites. Preferred Qualifications/ Transferable Skills*: You do not need to have the following preferred qualifications/ transferable skills to qualify. However, keep in mind we may consider some or all of the following when identifying the most qualified candidates. Licensed Board of Oregon (LPC, LCSW, PHD,etc). Knowledge of challenges and barriers faced by those with Severe and Persistent Mental Illness (SPMI) and the systems they encounter in Residential Services, Adult Protective Services and Crisis Services.
