• Has advanced Heart Failure
• Has developed some cognitive impairments (delirium). Sometimes she can't tell you what day it is.
• Doing speech therapy to help with cognition.
• Needs assistance with all ADLs
• Assistance with eating and meal preps
• Can scoot out of bed with assistance .Can stand and walk a few steps with a walker. Everyday she is gaining strength.
• Can pivot, but it's not easy for her to walk.
