```Duties```
- Provide personal care and assistance to hospice patients in their homes or in a nursing home setting
- Assist patients with activities of daily living, including bathing, dressing, grooming, and toileting
- Prepare meals and assist with feeding as needed
- Administer medication under the direction of a nurse or healthcare professional
- Monitor and record vital signs, such as blood pressure, pulse, and temperature
- Assist with mobility and transfers, using proper lifting techniques and equipment
- Provide companionship and emotional support to patients and their families
- Maintain a clean and safe environment for patients
```Experience```
- Previous experience in memory care or working with patients with dementia is preferred
- Experience in cooking and meal preparation is a plus
- Familiarity with home care or nursing home settings is beneficial
- Knowledge of catheter care procedures is desirable
- Ability to accurately document patient care activities in electronic systems
