As an Advanced Practice Provider at AllCare Primary & Immediate Care, you will work in a friendly and supportive environment, delivering high-quality medical care to our diverse patient population. You will be responsible for seeing bread and butter primary care patients, managing a varied caseload, and providing comprehensive healthcare services.

Location:

• Lorton, VA 22079

Schedule:

• 3 days a week: Monday to Friday (12-hour shift)
• Every Other Weekend: Saturday and Sunday (5-hour shift)

Responsibilities:

• Conduct comprehensive assessments of patients' health status.
• Develop and implement individualized care plans for patients.
• Provide primary care services, including diagnosis, treatment, and management of acute and chronic illnesses.
• Collaborate with interdisciplinary teams to coordinate patient care and ensure continuity of care.
• Provide patient education on disease prevention, health promotion, and self-care management.
• Participate in discharge planning and transitional care for patients.
• Prescribe medications and monitor psychopharmacology treatment.

Qualifications:

• Hold a valid Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of Virginia
• Have at least 1 year of employment experience working in a Primary Care/ Urgent Care setting.
• Strong communication skills and the ability to work collaboratively with a multidisciplinary team.
• Master's degree in nursing from an accredited program.
• Strong clinical assessment and diagnostic skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and collaboratively within a healthcare team.
• Proficiency in utilizing electronic medical records and other healthcare technology.
