We seek an organized and compassionate Care Manager for the Bethlehem/Northampton area to join our award-winning team. In this role, you can make a significant difference in the lives of our beloved seniors and caregivers. As a Care Manager, you will work closely with clients to identify their needs and goals, develop and implement action plans, and provide ongoing support and monitoring to ensure progress. The successful candidate will have strong communication and problem-solving skills and be able to work collaboratively with other team members. Responsibilities:

  • Leadership and Management accountability LMA for your Angels team (caregivers)
  • You will be responsible for handling leads (intake calls), meeting with potential clients, and, once onboarded, acting as the main point of contact for the client, family members, or client representatives, communicating all schedule changes.
  • Work with clients to identify their needs and goals and create action plans for clients to reach their potential needs.
  • Provide ongoing support and monitoring to ensure progress, including conducting follow-up meetings, check-in calls, and visits.
  • Building a relationship by matching client and caregiver needs.
  • Maintain accurate and up-to-date records of client interactions and progress.
  • Collaboration is key to our success, and as a Care Manager, you will play a crucial role in ensuring continuity of care and services. Your ability to work collaboratively with other team members will be highly valued and will contribute to the overall success of our team.

Qualifications:

  • We are looking for candidates with a minimum of five years of experience in case management, leadership, or a similar role. A growth mindset and a commitment to personal development are also essential for this position.
  • Strong communication and problem-solving skills
  • Ability to multitask and handle a high-volume workload.
  • Have the ability to follow through completely with tasks.
  • Ability to work collaboratively with other team members
  • A valid driver's license and reliable transportation are a must
  • Must report to the office daily
  • Intermediate computer skills in Microsoft Office products, Google Workspace, and navigating our scheduling software systems.
  • Must be able to sit for long periods, standing, bending, reaching, stretching, stooping, walking, climbing stairs, and moving intermittently during working hours.
  • Must be able to lift 50 lbs
