Individual Advocacy Group is Hiring a Registered Nurse (RN) Near Romeoville, IL

Position Summary:

To implement medical services, health support, continuity in nursing services, and meet documentation requirements to meet personal needs and DHS health regulations.

Duties:

Person Served:

  • Reads and checks the calendar schedule for person medical, health, and dental appointments assuring transportation, staff accompaniment and cross-checking with assigned QIDP, writing in both calendar and person chart disposition, and arranging for future appointments for services as needed by person need and IAG procedures to ensure health, safety, and welfare of persons.
  • May need to administer medications as needed at person located in the absence of DSP following written physician's order, verbal physician's order with follow-up written order and by person's ISP, writing in person's chart when administered and following IAG documentation procedures to meet emergency medical incidents and to meet IAG, District, and federal guidelines.
  • Reviews and comments on documentation regarding person injuries or illness occurring at a day program, school program or offsite facility activities in writing, recording current medical or mental status based upon monitoring after the incident by IAG incident procedure and to provide continuity in nursing services and to meet IAG, District, and federal guidelines.
  • Trains direct service staff and QIDPs in delivering health support services according to persons’ ISP, best practices procedures, and person-specific effective methods or techniques to provide continuity in nursing services and meet persons’ health, safety, and welfare.
  • Delivers health support services to persons according to an individual's program plan and prescribed methods and techniques following professional best practices principles to meet individual needs and IAG, District, and Federal guidelines.
  • Reads and checks daily appointment calendar for due dates of person's annual physicals, informing person's QIDP, appropriate staff, and related programs (e.g., DT school) of appointment commitment by daily schedule
  • Schedules and confirms non-routine laboratory work and tests as ordered by a physician, laboratory work, or test in person chart to provide necessary health care to persons and meet IAG, District, and Federal guidelines.
  • Reads and reviews laboratory and test results are reporting in writing or verbally to physician unusual or unexpected results to provide timely health care to persons and meet IAG, District, and Federal guidelines.
  • Reads and reviews medication orders, checks accuracy against physician's orders, reports variances in writing to physician and or pharmacy by IAG procedures, maintains safe and accurate health services and meets IAG, District, and Federal guidelines.
  • Reads and reviews physician's order sheets, checking for accuracy and content in preparation for physician's signature to ensure accurate documentation and meet IAG, District, and Federal guidelines.
  • Reads and reviews new medication sheets to periodically check the accuracy of listed medications against physician's order by IAG documentation procedure and meet IAG, District, and Federal guidelines.
  • Reads and reviews current medication sheet checking for nurse's initials against medication administration and medication changes against physician's order by IAG's documentation procedures and to meet IAG, District, and Federal guidelines.
  • Transcribing physicians’ orders, recommendations, and comments to maintain personal health and meet IAG, District, and Federal guidelines.
  • Transcribes physician's orders for medication or treatment changes and reevaluation on medication sheet by IAG procedures to meet IAG, District, and Federal guidelines.
  • Contacts pharmacy-using telephone placing orders or reorders (as needed or prescribed) for prescriptions following physician's orders on medication sheet to meet persons' health and medication requirements.
  • Signs and initials mediation record form following administrator of medication to a person keeping an accurate record of medication delivery to meet IAG, District, and Federal guidelines.
  • Checks and monitors persons in caseload for changes in physical or mental status, comparing current status against reference status recording in writing changes in person chart to maintain timely and efficient health care.
  • Contacts physician, QIDP, and family, and guardian using telephone or personal contact for any acute changes in-person physical or mental status by IAG procedures to provide timely and efficient health care and meet IAG, District, and Federal guidelines.
  • Provides documentation following periodic schedule documenting physical and mental status in diet and appetite, treatment, response and results, response to medications, etc., to provide continuity in nursing services and meet IAG, District, and Federal guidelines.
  • Provides documentation describing and monitoring particular medical problems according to documentation procedures to provide continuity in nursing services and meet IAG, District, and Federal guidelines.
  • Provides documentation of persons who are exhibiting current medical problems indicating physical and mental status by documentation procedures to provide
  • Continuity in nursing services and meeting IAG, District, and Federal guidelines.
  • Administers treatments and procedures following physician's order documenting treatment response in periodic notes according to IAG nursing and documentation procedures to meet IAG, District, and Federal guidelines.
  • Administers treatments and procedures for specialized services according to the individual's health and program plan, assuring intensity and frequency of intervention prescribed by the plan to meet personal needs and District technical services regulations.
  • Talks with and describes to staff current census writing and recording it in shift report indicating to shift nurse any changes in-person physical and mental status by IAG documentation and nursing procedures and to meet IAG, District and Federal guidelines.
  • Writes documentation of incidents resulting in injuries to persons, staff, or visitors, recording and filling out appropriate forms according to IAG procedure and meeting IAG, District, and Federal guidelines.
  • Writes nursing notes that person is away from the facility, except for scheduled events, activities, or programs (e.g., DT, school, routine facility off-site program) indicating that person is out, whom the person is with, whether the medication was sent with the person and also assuring person appropriately signed out by guardian and self by IAG procedures for documentation and sign-out procedures to meet IAG, District, and Federal guidelines.
  • Observe and physically check persons on caseload identified as ill, deliver nursing services following physician's order, document observations, notify the physician in the event of acute change in status by IAG procedures to provide continuity in nursing services meet IAG District and Federal guidelines.
  • Performs miscellaneous duties to ensure the unit's day-to-day functioning.

Job Type: Full-time

Pay: $75,000.00 - $80,000.00 per year
