Position Overview:

Are you ready to lead transformative change in the realm of Developmental Disabilities Health Home (DD HH) programs? Abilities First is seeking a dynamic and strategic-minded Director to spearhead our DD HH initiative. As the Director, you'll be at the helm of strategic planning, growth, and the overall administration of the program. Your mission? To ensure that every individual enrolled in our DD HH program receives exceptional care and support.

Key Responsibilities:

· Strategic Leadership: You will be the driving force behind the growth and development of Abilities First's DD HH Program. From formulating strategic plans to overseeing day-to-day operations, you'll play a pivotal role in shaping the future of the program.

· Team Empowerment: Lead, mentor, and inspire a dedicated team of professionals, including Nurse Care Managers and Facilitators. Your leadership will foster a culture of excellence and innovation within the DD HH program.

· Practice Transformation: Champion practice transformation within the DD HH program, promoting the integration of physical and behavioral health, prevention, and wellness. Your innovative ideas will shape the future of healthcare delivery for individuals with developmental disabilities.

· Contract Management: Collaborate with executive leadership to secure and retain contracts for essential healthcare consultants. Your attention to detail and negotiation skills will ensure alignment with DD HH requirements and uphold Abilities First's quality standards.

· Data Management: Ensure accurate and timely reporting of program data to regulatory agencies. Your meticulous oversight will guarantee the reliability and validity of all submitted data.

· Continuous Improvement: Implement strategies for continuous improvement within the DD HH program. From analyzing data to identifying areas for enhancement, your proactive approach will drive positive change and enhance program effectiveness.

Qualifications:

· Educational Background: Registered Nurse preferred, or a bachelor's degree in a related field with considerable experience.

· Experience: Previous experience in healthcare management preferred, with a demonstrated understanding of developmental disabilities.
