Abilities First is seeking a full time Licensed Practical Nurse responsible for supporting the healthcare needs of individuals within the Residential Services Program.

Position Summary

The Residential LPN will provide individual nursing care under the direction of the RNs, assisting with

Position Standards

· Monitors the health needs of participants by assuring all medical needs receive follow up.

· Maintain all medical information and records, ensuring records are current. Including review of MARs, Bowel Charts, Blood Sugar Flow Sheets, and Weight Records.

· Write nurse’s notes as required

· Perform medical filing per the direction of the Registered Nurses.

· Monitors the proper storage and handling of medications throughout the residential program, including disposal of empty blister packages and checking OTC medications for expiration.

· Complete medication orders and ensure formularies are in place for all medications.

· Verify doctor’s order in place for each medication, treatment, and adaptive equipment needs.

· Attends medical appointments of individuals as needed, completing the medical consult documentation.

· Administers medications to those individuals who are not self-medicating as needed.

· Order supplies as needed including med cups, gloves, etc

· Conduct medication regimen reviews and self-medication assessments under an RN’s guidance.

· Will assist the Nursing Coordinator with health care policy development and implementation.

· Counsel participants and staff regarding health maintenance and provide training where appropriate.

· Serve as a liaison to community resources such as physicians, labs, and day programs.

· Perform other nursing tasks as allowable under OPWDD’s Scope of Practice for LPNs (under the direction of an RN) after training and competency is demonstrated.

· Adhere to agency Code of Legal and Ethical Behavior, which require all employees, consultants, board members, volunteers and affiliates to perform their responsibilities according to ethical and legal standards with honesty, integrity, fairness, good faith and respect for others and the law.

· Follows all agency policy and procedures and demonstrates continuous regard to personal safety and the safety of others.

· Performs other appropriate job related activities as requested by your supervisor or as circumstances warrant.

Have the ability to lift up to 50 lbs., bend, squat, bend the knees, pivot and grasp with both hands (in order to properly and safely perform the techniques taught in CPR/First Aid, behavior intervention techniques, and lifting and carrying techniques).

Lift individuals by using appropriate techniques, including but not limited to usage of tools to assist, such as the Hoyer Lift.

Assist individuals in ambulation by providing physical assistance, including use of a gait belt or other equipment where necessary.

Have the ability to stand and walk for extended periods of time.

Have the ability to push, pull, and maneuver adults in wheelchairs.

Possess verbal and written communication skills, in English, to ensure adequate regulatory documentation.

Schedule: Monday - Friday, day shift with flexibility

Requirements

  • Must be licensed as an LPN by the NYS Department of Education.
  • Previous experience working with individuals with developmental disabilities is preferred.
  • Computer skills required.
  • Possess verbal and written communication skills in English to ensure adequate regulatory documentation.
  • Have the ability to stand and walk for long periods of time.
  • Have the ability to push and pull adults in wheelchairs, lift, bend, squat, turn in the knees, pivot, and grasp with both hands (in order to properly and safely perform the techniques taught in CPR/First Aid, behavior intervention techniques, and lifting and carrying techniques), with or without a reasonable accommodation.
  • Valid NYS driver's license deemed acceptable by our insurance carrier and ability to visit local residences as needed.

Job Type: Full-time

Pay: $25.75 - $27.75 per hour
