Job Title/Position: Patient Care Manager

Reports To: Clinical Director

JOB DESCRIPTION SUMMARY

The Patient Care Manager is responsible for ensuring that patient care is coordinated and managed appropriately. The Patient Care Manager is responsible for ensuring that care and services are delivered appropriately and for the supervision of clinical personnel.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

Clinical oversight of assigned team. Establish & maintain positive relationships with managed care team and community partners.

· Supervision, orientation, mentoring, developing and on-going evaluation of all patient care staff in specified teams, who are engaged in the delivery of care.

· Assures compliance with agency policies and procedures, state and federal laws, regulations, and accreditation standards and maintains compliance to the CHAP Hospice Care Standards.

· Participation in the Administrator On-Call (AOC) rotation.

· Assist with talent acquisition to ensure adequate staff is maintained.

· Coordinate and assist with facilitation of IDG meetings.

· Coordination and facilitating monthly department meetings as assigned.

· Assists QAPI with chart audits, clinical authorization review, and auditing compliance for patients.

· Works with the Clinical Director and staff clinicians to gather, collate, and report patient care summary information to Case Managers.

· Regularly participates in group meetings and assures through attendance and record reviews that meetings are conducted in accordance with Agency and CMS regulations.

· Maintains comprehensive working knowledge of patients and ensures the needed documents are reviewed and organized in the system.

· Works with all members of the clinical team to ensure that patients are following physician plan of care orders.

· Assists in the development and organization goals. Develops, recommends, and administers organization policies and procedures.

· Performs other duties and activities as delegated by the Clinical Director.

· Will be expected to be back up to admissions, case management, and triage visits as needed.

· Participates in community educations and provides adequate training and community partners.

POSITION QUALIFICATIONS

1. Registered Nurse with current licensure to practice professional nursing in the state of Arizona.

2. Graduate of ACEN or CCNE accredited school of nursing.

3. Previous experience in hospice/home care setting with two (2) years management or supervisory experience. Proven ability to work within an interdisciplinary setting.

4. Complies with accepting professional standards and practice.

5. Has excellent observation, good nursing judgement and communication skills. Understands hospice philosophy and issues of death/dying.

6. Understands principles of pain/symptom management.

7. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

8. Complies and maintains current CPR certification.

9. Demonstrates excellent observation, verbal, and written communication skills.

Job Type: Full-time

Pay: $85,000.00 - $90,000.00 per year
