Job Title: Care Manager
Reports to: Program Manager - West Side Project
Full Time
Located in the Upper East Side, Search and Care seeks out older adults in the community who need help managing life’s daily activities or accessing essential services. Search and Care provides clients with support and companionship to help them remain independent and safe in their homes. Search and Care currently serves adults over 60 in the Manhattan neighborhoods of Yorkville, Upper East Side, East Harlem, and the Upper West Side.
Position Overview:
The Care Manager has an awareness of the biological, psychological, and social impact that disabilities, illnesses and aging can have on the population that Search and Care serves. Under the supervision of the Project Manager, the Care Manager will conduct assessments and will provide assistance with benefits/entitlements applications, referrals, and development of safety and service plans with clients according to NYC Aging guidelines. The Care Manager will oversee an assigned caseload of older adult clients and will remain in consistent contact with them to ensure they remain safely independent with an acceptable quality of life.

Salary Range: $58,000-62,000 depending on credentials and experience.
