New Directions Northwest is Hiring an Intake Specialist/AIC Care Coordinator Near Baker, OR

General Purpose: Employee of New Directions Northwest (NDN) a non-profit agency working at Powder River Correctional Facility New Directions Program, located in Baker City. The Intake Specialist/AIC Care Coordinator is responsible for maintaining accurate waitlist, performing intake duties for new AIC's entering the program, coordinating and conducting UA's, coordinating graduations, maintaining MOTS/ROADS and assisting with maintaining accurate client records in EHR for the Powder River Correctional Facility Alternative Incarceration Alcohol and Drug treatment programs. Confidentiality is required to carry out special timely projects.

Supervision Received: Intake Specialist/AIC Care Coordinator works under direct supervision of the Program Director.

DOC Expectations: The Oregon Accountability Model (OAM) depends upon a team approach to custody with the security-series personnel and non-security personnel communicating effectively to maintain safe, secure, and sanitary correctional environments while preparing Participants for successful reentry into the community. The vast majority of DOC Participants will return to Oregon communities. Enhanced public safety, lower crime rates, and reduced recidivism can all be tied directly to attitudes and actions of correctional staff who must effectively apply ODOC rules and transition initiatives in their daily interaction with Participants. All staff are expected to model pro-social behaviors; reinforce positive behaviors; redirect negative behaviors; and effectively communicate observed behaviors to responsible program staff, administrators, and supervisors in support of OAM.

ESSENTIAL DUTIES AND RESPONSIBILITIES

This Position requires:

  • Excellent multi-tasking and very detailed orientated skills.
  • Create and maintain an accurate waitlist
    • Create records for potential AIC's
    • Gather and enter demographic information
    • Research LS/CMI information and any other pertinent information from DOC 400 and other resources
  • Conduct Intake meetings and Complete Intake Process
    • Enroll/Admit AIC in AIP Program
    • Review Program Rules and Expectations
    • Complete and Review Intake packet in entirety
    • Review Client Rights
    • Review Grievance Process
    • Review and sign Consent to Treat
    • Complete ROI's for:
      • DOC - Medical
      • DOC - Treatment Administration Team
      • DOC - Superintendent of PRCF
      • Emergency Contact
      • Any others that are applicable
    • Administer and Score Intake TCU Test
    • Entrance UA
    • Follow direction of Office Support Specialist
  • Maintain TCU Testing requirement schedule
    • Administer Tests
    • Score Tests and Enter scores in EHR
  • Maintain and enter MOTS/ROADS data in a timely manner (both entrance and discharge)
  • Organize and attend AIC Graduations for both units
  • Arrange and schedule AIC Presentations when requested
  • Coordinate and facilitate GP Outreach modules per Program Director Instruction
  • Administer and document all UA's
    • Entrance, Random and Exit UA's
    • Provide training for other male NDN staff on UA process, to provide assistance/coverage when on vacation, sick and other times of need.
  • Communicate with DOC CO's each morning to get a list of requested lay-ins and document them each day in EHR.
  • Arrange for completion of Exit Surveys
  • Arrange, document and communicate After Care appointment
  • Assist the Office Support Specialist with Scanning and uploading the following items into the EHR 'Documents Accountabilities' Folder
    • Misconduct Reports
    • Staff Sanctions
    • Conduct Orders
  • Enter all Daily Performance Failures in EHR
  • Complete and Maintain a reference manual that provides step by step instruction on all duties described in job description
  • Complete Addiction Assessments on new admits when requested by Program Director
  • Cover/Facilitate Groups when requested by Program Director
  • Other duties as assigned

QUALIFICATIONS

  • High school diploma or GED. Valid Oregon Driver's License
  • 2 years' experience in office secretary or office assistant
  • Excellent typing skills, (above 50 words per minute)
  • Excellent computer skills
  • Communication skills/public relation skills
  • Maintain a positive attitude
  • Complete tasks on time
  • Able to multi-task
  • Capable of independent organization and prioritizing workload

TOOLS AND EQUIPMENT USED

Mobil two-way radios, computers and software, copy/fax/scan machine, telephone, Electronic Health Record system, Paychex scan machine.

Specific vision abilities required by this job include close vision and the ability to adjust focus.

WORK ENVIRONMENT

The working environment is representative of those New Directions Northwest, Inc. employee encounters while performing the essential functions of this job in a prison. The position required the employee to dress in a professional manner that meets the standards for professional correctional counseling employees. Preferably a shirt and tie, non-revealing dress, slacks, good sturdy shoes. No tee shirts or blue jeans will be worn during the performance of this job.
