Seeking Full-Time Community Hospice Registered Nurse
$7,500 Sign on bonus if hired!
Job Summary: Hospice of New York is seeking a dynamic Registered Nurse to provide continuous nursing assessment, planning, monitoring, coordination, and implementation of care to maximize the comfort of patients with a life-limiting illness, while supporting their family and caregivers. The Hospice RN collaborates with other members of the Interdisciplinary Team to create an individualized Plan of Care to meet the physical, medical, psychosocial, emotional, and spiritual needs of the patient and family unit.
Responsibilities:
1. Completes initial and ongoing patient/family assessments including physical, psychosocial, spiritual, and bereavement to provide palliation of end-of-life symptoms and stressors in collaboration with the Interdisciplinary Team, as assigned.
2. Continuously updates the Hospice Plan of Care to reflect the patients’ current conditions and needs based upon the initial and all subsequent assessments.
3. Professionally manages and delivers patient/family care in accordance with the Hospice Plan of Care.
4. Documents all visits and changes in condition or care at the time of event and location of care, unless otherwise directed.
5. Identifies and utilizes resources for safe, effective, comprehensive palliative care management such as Pharmacy, Durable Home Medical Equipment, Supplies, Therapy, Hospice Aide services, and physician consultation.
6. Initiates communication with attending physicians, other hospice staff members, contract providers, and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
7. Provides education and training to patients, families, and caregivers in the care of the patient, including pain and symptom management.
8. Ensures the Hospice Plan of Care integration with Skilled Nursing Facilities and Assisted Living Facilities.
9. Serves as a patient and family advocate.
10. Provides exemplary Customer Service.
Qualifications:
o Registered nurse currently licensed in New York State.
o BSN degree in nursing from an approved program; or Registered professional nurse with two years’ experience in either home health care or Hospice care.
o Knowledge of Computer operation, computer generated clinical records experience preferred.
o Demonstrated commitment to Hospice philosophy of care.
o Unless ALL assigned patients can be served by using public transportation, must have reliable automobile, valid driver’s license, and the minimum state required liability automobile insurance.
