Good Samaritan Hospice, a mission of Concordia Lutheran Ministries, provides compassionate home care, facility based, and inpatient hospice services to our patients and their families. Our home care coverage area spreads throughout several counties in Western Pennsylvania with inpatient locations in Wexford, Cabot, and Beaver. The Good Samaritan Hospice team works together to provide relief through pain control and symptom management, while offering emotional, social and spiritual support for the patient and family. POSITION SUMMARY
The Spiritual Care Coordinator is responsible for assessing the spiritual needs of hospice patients and providing spiritual care for the patient and family directly or with other spiritual care providers.
QUALIFICATIONS
- Bachelors degree in counseling or graduate of accredited seminary or school of theology
- Masters degree in associated field preferred
- Ability to work in an interfaith setting with different lifestyles, cultures, beliefs and values
- Solid computer skills, including Word
- Excellent organizational skills with attention to detail
- Outstanding communication skills (both written and verbal)
- Reliable independent means of transportation with a valid Pennsylvania driver’s license and at least the minimum state required vehicle insurance
RESPONSIBILITIES
1. Coordinates the spiritual care of patients/families by working closely with the Family Support Services Assistant to ensure appropriate support is offered and available in accordance with the Good Samaritan Hospice Mission Statement, company values and hospice/government regulations
2. Conducts initial and ongoing spiritual care assessments for the patient and family in a timely and supportive manner
3. Works with the Interdisciplinary Group (IDG) to coordinate the spiritual care for hospice patients/families
4. Works closely with the bereavement team to identify grief and bereavement needs and establish a plan of care
5. Maintains regular communication with patients, families, IDG members and others involved in patient care
6. Provides consultation, education, and spiritual/bereavement care and support to patients and family members
7. Coordinates spiritual care with other spiritual care providers as necessary
8. Attends and participates in IDG meetings
9. Attends and participates in staff meetings and other related activities when required
10. Completes required in-services; ensures all required continuing education credits are obtained for licensure renewal
