General Summary:
Community-Based Case Managers needed in the Southern Maine region.
The primary responsibilities of the Case Manager are to provide comprehensive case management services throughout the Community Integration, Behavioral Health Home, Opiate Health Home, and Targeted Case Management programs. The Case Manager’s responsibilities include working with a team to develop, implement and monitor service plans; develop short- and long-term goals; develop resources and assist in placement; identify, coordinate and advocate for provision of services; and prepare, maintain and monitor case plans and records. The Case Manager will be required to maintain a billable expectation once their caseload is built. The Case Manager must be willing to assume responsibility for functioning as a consultant, educator and advocate. Work is reviewed through reports, observations, supervisory evaluation and analysis of results obtained.

On an ongoing basis strictly adheres to all Day One, Federal and State policies and procedures regarding the confidentiality of client, employee and Agency information.
Supervisory Responsibilities:
None
Physical Environment:
Office based duties can be performed out of any of our office locations in Cumberland County. Travel within that community is required as needed to meet the needs of the clients we serve.
Physical Effort:
