General Summary:

Community-Based Case Managers needed in the Southern Maine region.

The primary responsibilities of the Case Manager are to provide comprehensive case management services throughout the Community Integration, Behavioral Health Home, Opiate Health Home, and Targeted Case Management programs. The Case Manager’s responsibilities include working with a team to develop, implement and monitor service plans; develop short- and long-term goals; develop resources and assist in placement; identify, coordinate and advocate for provision of services; and prepare, maintain and monitor case plans and records. The Case Manager will be required to maintain a billable expectation once their caseload is built. The Case Manager must be willing to assume responsibility for functioning as a consultant, educator and advocate. Work is reviewed through reports, observations, supervisory evaluation and analysis of results obtained.

Essential Duties and Responsibilities:

  • Develop an Individual Service Plan (ISP) with each enrolled youth through intake, assessment, treatment, and on-going meetings. This includes goals and needs for the seven areas of domain.
  • Develop, monitor and modify individual service plans for clients and their families.
  • Discuss progress with clients and others involved in the service plan, advise clients on responsibilities and progress towards ISP.
  • Assist clients and/or their family in identifying appropriate service providers, advocate and refer for services, and coordinate the provision of services with agencies and providers.
  • Meet regularly with clients.
  • Review and monitor case records, prepare regular and special reports and summaries on clients progress in a timely basis. Record and report unusual situations or incidents that may affect client’s placement and participation in the program to agency supervisory and program coordinator, maintain responsibility for timely case file management.
  • Maintain all clinical records in accordance with agency, program, licensing and clinical standards.
  • Provide timely statistics and client data necessary to fulfill contract and billing requirements.
  • Participate in clinical supervision, training and staff meetings.
  • Represent Day One professionally in the community.
  • Performs all other duties as assigned.

Physical Environment:

Office based duties can be performed out of any of our office locations in Cumberland County. Travel within that community is required as needed to meet the needs of the clients we serve.

Physical Effort:

Requirements and Skills Needed for Position:

  • MHRT-C Certification
  • LSW or LSX preferred
  • Experience working with individuals, of all ages, with mental heath and/or Substance use concerns.
  • Ability to consistently work collaboratively and effectively as a team member.
  • Good verbal, written and interpersonal communication skills.
  • Reliable transportation and ability to travel locally.
  • Ability to work flexible hours as needed to meet needs of clients.
  • Ability to cope with stressful situations and to interact cordially with clients and co-workers to accomplish tasks.
