Responsibilities:
-Assist Administrator with admission process- preparing necessary paperwork
Apply for Medicaid Waiver application for residents
Apply for Social Security benefits if needed
Apply for Veteran Aid and Attendendes if qualifies
Apply for Cat D if needed
Provide case management services to individuals and families in need
- Advocate for clients and access necessary services
- Supervise and monitor client progress, making adjustments to service plans as needed
- Provide mentoring and guidance to clients, offering support and encouragement
Skills:
- Strong communication skills, both verbal and written
- Excellent organizational and time management abilities
- Ability to work effectively with diverse populations
- Knowledge of Medicare, Medicaid, VA, social security
- Proficiency in using computer software for documentation and record keeping
