City of Longview, TX is Hiring a Care Coordinator Near Longview, TX

Assists the EMS Section with the completion of needs assessment documentation and provision of support activities in accessing resources including housing, employment, basic needs, primary care and other natural support; encourages the use of home and community resources and continues to assess and follow up on individual’s needs; performs other related duties as assigned.

Examples of Duties

Coordinates with the community paramedic on high risk for hospitalization and works with the treatment team to implement a plan to prevent current or future inpatient care. Makes appropriate referrals and coordinates the delivery of services. Reviews the results of the assessment and/or the ongoing therapeutic assessment process in order to determine the ongoing needs of the individual. Works with assessment team to insure needs are being met and determines next course of action. Documents client needs which includes but not limited to release of information, service changes, activities, etc. as indicated by audits. Assists in the development of a care plan to include all service coordination and skills training activities to assist the individual in the resolution of identified problem areas. Demonstrates the ability to identify and prioritize a crisis situation. Creates and runs monthly reports for necessary regulatory agencies. Provides support in home and/or office setting as appropriate for target needs identified as deficiencies in assessment. Performs other related duties as assigned or required.

Minimum Requirements

Bachelor’s Degree in Social Work, Psychology or related field, and two years social work or case management experience; OR an equivalent combination of education and experience.Must possess a valid Texas Driver’s License.

Physical Demands/Work Environment

Work is typically performed in a standard office environment but could have exposure to inclimate weather and/or infectious diseases. Subject to sitting, standing, walking, bending, reaching, and lifting of objects up to 20 pounds.
